GRANT APPLICATION GUIDELINES  

Grantseekers who are first time applicants or those who have not been funded in the past must contact the Foundation’s Executive Director three months prior to the grant deadline in order to schedule a site visit. Grants will not be considered without a prior site visit.

Grant Deadlines: The first week of April and the first week of October.

Please provide the following information:

I. Cover letter. A one page letter to the Executive Director of the Jubitz Family Foundation summarizing the request. We do not accept requests for capital expenditures.

II. Grant Application Form. Note: This form should not be longer than two pages.

III. Additional Information.

A. Evaluation: What results or outcomes are expected? How can these be evaluated and measured? When do you propose that this evaluation be done?

B. Budget for the project: Please provide a brief budget and budget narrative for the entire project making explicit reference to the portion of the budget requested from the Foundation. Also, please indicate the organization's overall budget.

IV. Appendix.

A. Biographical sketch of principle contact person and relevant personnel.

B. Copy of organization's 501(c)(3) IRS status letter.

C. Organization's current audited financial statements. If an audit is not done the first page of the 990 should be included.

D. List of Board of Directors.

E. List of Major Donors.
